Collaboration with community agency partners results in opportunities to share ideas, resources, and information to improve post-school outcomes for transition-aged students with disabilities. This overview discusses the importance of effective collaboration, Individualized Education Programs (IEPs) and agency involvement, and provides practical tips and resources for parents, educators, and students.

LaQuenta Montañez supports students with disabilities, 14- 21 years of age, in transitioning to their desired post-school outcomes, whether postsecondary education/training, and/or competitive employment.  Utilizing high leverages practices (HLPs) and research-based instructional and work based learning experiences, it is the goal of the SDP to ensure that students with disabilities are college and career ready upon graduating from high school. It is LaQuenta’s passion to ensure that all students with disabilities are provided with transition services and activities in order to reap success, and lead productive lives in today’s society.
